APPLICATION PROCEDURES

1. Go to http://trece.cvsu.edu.ph/home/signup

2. Create your online account. 

3. Log in to your registered account and fill out the required details. In filling our the online form, remember the following: 

  • Do not edit (tamper) the information-data of your application form outside the online admission system.
  • The declared strand in the application form and the strand declared in the required document should match
  • Choose the correct entry type of applicant.
  • Thoroughly check all information provided in the online admission system before saving and submitting to avoid typographical errors, wrong spelling, incomplete, or wrong entry of information.
  • Once there is a control number, the information details cannot be edited again.

  • 4. Download the accomplished application form, attach your 1x1 ID picture, and affix your signature. 

    5. Submit your application form enclosed in a short brown envelope to the Campus Admission Office together with the following required documents/credentials: 


    DOCUMENTS/CREDENTIALS TO BE SUBMITTED:
    First year applicants

    For Grade 12 Students
  • Copy of accomplished application form for admission;
  • Photocopy of Grade 11 Report Card;
  • Certificate from the Principal or Adviser indicating that the applicant is currently enrolled as Grade 12 student and the strand is also indicated. The certificate must be originally signed. E-signature is not allowed.
  • Short brown envelope
  • 2pcs 1x1 ID picture, white background
  • Note: If BSP is the preferred course, your GPA must be 85 or higher.
  • For SHS Graduate
  • Copy of accomplished application form for admission;
  • Photocopy of completed Grade 12 report card;
  • Certification of non-issuance of Form 137 for College Admission (this certification shall prove that the applicant has never been enrolled in another university/college)
  • Short brown envelope
  • 2pcs 1x1 ID picture, white background
  • Note: If BSP is the preferred course, your GPA must be 85 or higher.
  • For ALS Passers
  • Copy of accomplished application form for admission;
  • Photocopy of Certificate of Rating (COR) with eligibility to enroll in College
  • Short brown envelope
  • 2pcs 1x1 ID picture, white background
  • For Associate, Certificate, Vocational, or Diploma Degree Holder
  • Copy of accomplished application form for admission;
  • Photocopy of Transcript of Records (TOR) with graduation date;
  • Short brown envelope
  • 2pcs 1x1 ID picture, white background
  • Transferee Applicants
  • Copy of accomplished application form for admission;
  • Photocopy of Transcript of Records (TOR) / Certificate of Grades (COR) signed by the school registrar
  • Other required documents like Honorable Dismissal, Certificate of Good Moral Character, NBI Clearance or Police Clearance will be submitted later on after the applicant passed the admission exam
  • Short brown envelope

  • Note: If COG attach contains failed or INC will be denied.
  • Second-course Applicants
  • Copy of accomplished application form for admission;
  • Photocopy of Transcript of Records (TOR) with graduation date
  • Other required documents like Honorable Dismissal will be submitted later on after the applicant passed the admission exam
  • Short brown envelope
  • 2pcs 1x1 ID picture, white background
  • 1. All transferee-applicants must undergo evaluation first before being schedule for the admission exam.
    2. Applicants will select from the admission system their preferred date of submission of their application.
    3. On the appointment date, applicant will submit their application form together with the complete required documents at the Campus Admission Office. Incomplete requirements will not be processed. Further, only applicants who are schedule on the particular date will be accommodated.
    4. Once the application has been received by the Admission Office, the applicant shall be schedule for admission exam. Examination permit will be issued to the applicant..
    NOTE: The following are strictly prohibited and will mean non-evaluation of application:
    1. Multiple applications in the online admission system. This can be traced by the evaluator.
    2. Creation and deliberately using different email addresses to access the online application form.
    3. Any deception, falsification, and dishonesty in the declaration of information and submitted requirements
    4. Submission of incomplete requirements.


    REMINDERS AND THINGS TO BRING DURING EXAMINATION

    1. Bring Pencil No. 2
    2. Attach 1x1 ID picture in the examination permit
    3. Bring ID Card, and alcohol/sanitizer
    4. Keep your examination permit safe and secure. No permit, no examination
    5. Do not be late. Be present at the examination venue 30 minutes before the time.


    HEALTH PROTOCOLS DURING THE ADMISSION EXAMINATION

    1. Only applicants who are schedule for admission exam shall be allowed to enter the Campus premises and will be directed to the designated area of examination. They will not be allowed to roam around the Campus.
    2. Physical distancing must be observed for the entire duration of the applicants’ stay in the Campus premises.
    3. Face mask must be worn properly at all times during the examination
    4. No food and drinks are allowed during the examination.


    EXAMINATION RESULTS

    The result of examination will be released tentatively on TBA through the CvSU Trece Martires City Campus website (cvsu-trececampus.com). The Office of Student Affairs and Services (OSAS) informs all applicants that filling-out the online application form and having a control number do not mean a sure slot for admission. All applicants are advised to remember the control number assigned to them.
